ADDITIONAL TERMS AND CONDITIONS
  1. This agreement is not transferable or assignable and applies only to the pet designated above while owned by the above client. Either AAH or the owner may cancel this agreement at any time. The owner may cancel for any reason (including but not limited to death or return/transfer of the pet). If the total amount of services rendered by AAH prior to cancellation (valued at AAH’s full retail prices) is less than the amount the owner has paid, a refund by AAH to the owner for that difference will be paid. If the total amount of services rendered (valued at AAH’s full retail prices) is greater than the total fee for the preventative care package, no refund by AAH to the owner will be made. If the owner performs all his or her obligations and AAH cancels the agreement, all fees the owner has paid for the plan term year will be refunded in full, less AAH’s full retail charges for any services rendered before cancellation. In no other cases will any amount paid by the owner be refunded upon cancellation.
  2. This agreement will be effective for the period stated above unless and until the owner or AAH terminates this agreement.
  3. The following items or services are NOT covered by this agreement: 1. Services rendered by specialists to whom the pet is referred by AHH. 2. Services at any institution or hospital, other than AAH, or services by veterinarians not employed by AAH.)
  4. In the event that cancellation of this agreement for any reason results in money due to either the owner or AAH, such money shall be paid in full within 30 days of cancellation. If either party fails to make any payment when due under this agreement, that party shall pay the other party’s collection costs whether or not legal action is commenced. In the event of any legal proceedings (including appeals), the prevailing party shall be entitled to recover its costs, disbursements, and reasonable attorney’s fees as determined by the court.

  • All regular office visits FREE during regular hours (emergency and/or urgent care office visits excluded)
  • ProHeart12 (12-month injectable heartworm prevention) OR 50% off a purchase of a 12 pack of Heartgard (choose one)
  • 50% off urgent care office visit fee and emergency visit fee at Andover Animal Hospital
  • 25% off a dental cleaning (cleaning fee only. Does not include extractions)
  • 20% off a spay or neuter

YOUR PREVENTATIVE CARE PLAN COVERS ALL THE SERVICES LISTED ON THIS PAGE FOR THE PERIOD OF ONE YEAR FROM THE START DATE. IT DOES NOT INCLUDE ANY SERVICES NOT LISTED HERE, ANY SERVICES PROVIDED BY ANYONE OTHER THAN ANDOVER ANIMAL HOSPITAL ("AAH"), OR ANY FEES FOR SERVICES RECOMMENDED AS A RESULT OF ILLNESS OR ACCIDENTAL INJURY.

  • I understand that this Wellness Plan is non-transferable and non-refundable.
  • I understand that emergency fees, urgent care fees, and drop-off fees are not included.
  • I understand that extractions are not included in the dental cleaning discount.
